Q: What about boxes 1 & 6? Do I need to fill these out?

A: No, we will fill these sections out with the correct information


Q: What if the notary will not sign it without all boxes filled?

A: Please tell the notary that boxes 1, 2 & 6 are specifically for the CMRA (Commercial Mail Receiving Agent) to fill out and that they should be left blank


Q: Can spouses fill out the same form?

A: No, each adult receiving mail will need to fill out a separate 1583 form.


Q: I’m overseas and don’t have access to an American Notary or an online notary service. What can I do ?

Q: Do I need to send my physical ID to you or just an image/scan?

A: Please do not send your physical ID to us. We only need a printed copy of the IDs or a scanned and emailed image


Q: Can I use an expired ID? 

A: All IDs used for the 1583 form must be current and unexpired. 


Q: Do I have to give Texas Home Base 2 forms of ID? 

A: Yes, the USPS requires us to receive 2 current forms of ID for all adults receiving mail at Texas Home Base and at any other CMRA (Commercial Mail Receiving Agent) in the US. 


Q: Do I need to sign the form?

A: Yes, please sign the form on line 13a and include the date in 13b. The USPS requires that you sign the form in the presence of a notary or by using an online notary service. 


Q: Who should sign 14a as the witness? 

A: The notary will be the witness for this form. 


Q: Why do the USPS instructions say "witness can be the agent, an authorized employee, or a Notary Public?" 

A: If you come to Wichita Falls and sign the form in the presence of one of our employees, we can sign the form as the witness. However, if you do not sign the form in the presence of one of our employees you will need to find an in-person notary or use an online notary service. 


Q: Will I ever have to complete another 1583? 

A: If one of your IDs expires, we will be notified by the USPS and will then contact you for the new information. You may need to sign a new form to complete this process. 


Q: Can I use 1530 P B Ln as my home address on the form? 

A: If the 1530 P B Ln address is on your ID(s) you will need to use it on lines 4f-4j & 9b-9f. The information on these lines should match the address on your ID(s). 


Q: Can I use my international address as my home address on the form? 

A: If the international address is on your ID(s) you will need to use it on lines 4f-4j & 9b-9f. The information on these lines should match the address on your ID(s). 


Q: Do lines 4f-4j need to match lines 9b-9f? 

A: Yes, these lines should match the address on your ID(s). 


Q: What is an "authorized individual?"

A: The "authorized individual" is a person that you want to authorize to pick up your mail in person at Texas Home Base. You are required to include their information in boxes 5, 10 & 11. You will also need to send us 2 forms of ID (one must be a photo ID) for all "authorized individuals." 


Q: Do I have to include an "authorized individual?" 

A: No, you do not. This section of the form is optional and can be left blank if you do not want to authorize anyone to pick up your mail. 


Q: Where can I include my minor children that might receive mail at my address? 

A: Please include the names of all minor children on line 12 "Exceptions for Additional Recipients of Mail."
