USPS 1583 FORM INFORMATION
The USPS 1583 form is primarily to ensure that no one is trying to steal your identity through your mail.
We are required to receive a USPS 1583 form for all adults receiving mail at our 1530 P B Ln, Wichita Falls, TX 76302 address. Each adult will need to fill out a single 1583 form. This means that spouses cannot fill out one form together, they will need to fill out separate forms for each person. If you have a business that uses this address, you will need to include the business's information on your 1583 form. If you have multiple businesses, you will need to fill out a separate 1583 form for each business name.
In order to use a CMRA (Commercial Mail Receiving Agent) in the US, you are required to fill out this form.
Since this is a government form, it can be a little confusing. You will find instructions below for completing the form and information on the ID requirements. Please feel free to contact us with any questions you may have regarding this process and/or the form. There are 3 sections of the form:
Section 1: Your information - in the past, spouses could complete a single 1583 form together, however with this new form (released April 2023) spouses will need to complete separate forms.
Section 2: "Authorized Individual" information (optional) - this is to authorize a separate person (sibling, parent, adult children, etc.) to pick up your mail at Texas Home Base.
Section 3: Business Information (optional), Your Signature & Notary's Signature (required)

Section 1: Your Information
You will put your information in boxes 4, 8 & 9. The address on lines 4f-4j & 9b - 9f should match the ID(s) you're using for the form.
You will select your types of ID in boxes 8e & 9g. At least one ID will need to include a photo. You cannot use the same ID for both boxes. At least 2 forms of ID are required for each adult using Texas Home Base or any other CMRA (commercial mail receiving agent) service. You can find the acceptable forms of ID in boxes 8e & 9g as well as the bottom of "Section 1."
The 2 forms of ID that you list on the form and send to us must be current and unexpired.
Lines 8a-8d are for the information from your first ID (identification options listed in 8e).
The USPS 1583 must be accompanied by 2 forms of ID for each adult receiving mail at your new address
Acceptable forms of ID include:

- Photo ID: U.S. State/Territory/Tribal Driver's or Nondriver's ID Card; Uniformed Service ID; U.S. Access Card; U.S. University ID Card; Passport; Matricula Consular; NEXUS Card; Certificate of Naturalization; U.S. Permanent Resident Card

- Second ID: U.S. State/Territory/Tribal Driver's or Nondriver's ID Card; Current Lease; Mortgage or Deed of Trust; Home or Vehicle Insurance Policy; Vehicle Registration Card; Voter Card
Section 2: "Authorized Individual" (optional)
This section is to authorize a person (sibling, parent, adult child, etc.) to pick up your mail at Texas Home Base.
In this example, we are authorizing John Doe's sister Jane Doe to pick up his mail.
This section is not for spouses to be included on the account. All adults receiving mail at Texas Home Base (including spouses) will need to fill out an individual USPS 1583 form. Spouses cannot sign the same form.
If you do not want to authorize anyone else to pick up your mail in person, leave boxes 5, 10 & 11 blank.
You will put the "authorized individual's" information in boxes 5, 10 & 11. The address on lines 5d-5j & 11b-11f should match the ID(s) your "authorized individual" is using for the form.
You will select the types of ID for the "authorized individual" in boxes 10e & 11g. At least one ID will need to include a photo. You cannot use the same ID for both boxes. At least 2 forms of ID are required for each person you authorize to pick up your mail on your behalf. You can find the acceptable forms of ID in boxes 10e & 11g as well as the bottom of "Section 1."
The 2 forms of ID that you list on the form and send to us must be current and unexpired.
Lines 10a-10d are for the information from the "authorized individual's" first ID (identification options listed in 10e).
Section 3: Business Information (optional), Your Signature & Notary's Signature (required)
Since this form is primarily to ensure that no one is trying to steal your identity through your mail, the USPS requires the 2 forms of identification as well as a Notary to verify that you have signed the form. We understand that this may be an inconvenience, especially if you are overseas. We ask that you try your very best to find and use a notary (or online notary service) for the signature verification portion, however if you are unable to find a notary and unable to use an online notary please contact us at accounts@texashomebase.com or (866) 766-2095. We answer our phones from 8:30 am - 2:30 pm CST Monday through Friday. If you call outside of these hours, please leave a message and we will get back to you as soon as possible.
Business Information & Minors (optional)
Box 7 includes the business name, business type, the official street address , the phone number and the County & state of registration (or where it’s “registered agent” is located)
If the business has any other partners or managers that may receive mail periodically, please list that information on line 12 “Exceptions for Additional Recipients of Mail”
If you have minors in your household that will receive mail at Texas Home Base, please include them on line 12.
Signature & Notary (required)
To complete the form, the USPS requires that you sign it in front of a notary or by using an online notary service. You will sign on line 13a and the notary will sign on line 14a.
There is also an optional Notary seal area on the bottom of page 2 if the Notary does not use their own acknowledgement form.
Boxes 1, 2 & 6
Please leave these boxes on the form empty. We will fill them out at our office as needed.
Sending the Form to Texas Home Base
You will send the form directly to Texas Home Base once it's completed and signed. The USPS requires that each CMRA (commercial mail receiving agency) submit the forms with their new system.
If you sign the form with an ink pen, you will need to send it to our address - 1530 PB Ln, Wichita Falls, TX 76302.
If you use an online notary, you can send the completed and signed PDF to - accounts@texashomebase.com.
You can print an image of the 2 forms of ID for each adult and send the image(s) in the mail with your 1583 form OR if you don't have a printer or prefer to send them digitally, you can send the image(s) via email - accounts@texashomebase.com.
New Applicants:
- If you can send proof that the physical form was sent to Texas Home Base with a tracking number before 4:30 pm on a business day, an address will be sent to you that day. If we receive the tracking number after 4:30 pm or on a weekend, an address will be sent the next business day. You can send the tracking number to accounts@texashomebase.com.
- If you do not send us a tracking number, the address will be sent as soon as we have received the physical form.
