NEW APPLICANT INFORMATION
As a reminder to complete a new application you must download the USPS Form 1583.pdf, fill it out and send the physical form to us via USPS, FedEx or UPS. To send us a copy of the 2 forms of identification required (one must be a photo ID), you can either send a printed copy of them with your form or you can send them via email to accounts@texashomebase.com. Click here for instructions on completing the 1583 form.
If you can send proof that the physical form was sent to Texas Home Base with a tracking number before 4:30pm on a business day, an address will be sent to you that day. If we receive the tracking number after 4:30pm or on a weekend, an address will be sent the next business day. Please send the tracking number to accounts@texashomebase.com.
If you do not send us a tracking number, the address will be sent as soon as we have received the physical form & copy of your IDs.
