PDF Form 1583
In order to receive a new address you must download the United States Postal Service PS Form 1583, fill it out and then upload this form by clicking the "START UPLOAD" button below.
You must include a copy of two forms of ID, one of which must be a photo ID. Acceptable and unacceptable forms of ID are listed on line 8 of the USPS 1583 form.
Spouses may complete one USPS 1583 form with both names listed on lines 2 & 6. We will need 2 forms of ID for each spouse (one photo ID and one secondary ID).
Any other adult(s) receiving mail at your new address must complete a separate USPS 1583 form and submit 2 forms of ID (one photo ID and one secondary ID).
Social Security cards, credit cards, and birth certificates will not be accepted.

An address can not be sent to you until Texas Home Base has received this form.
Max Photo ID upload 3MB. Photo ID must be .jpg format.
Max Secondary ID upload 3MB. Secondary ID must be .jpg format.
