Our ID Upload page is currently under construction. You can send us your IDs either via email to dru@texashomebase.com or you can send a paper copy of the IDs with your USPS 1583 form. You will find more information about this form & IDs required below:
In order to receive a new address you must download the United States Postal Service PS Form 1583, fill it out and forward the physical form to: Texas Home Base, 1530 PB Ln, Wichita Falls, TX 76302.
- If you can send proof that the physical form was sent to Texas Home Base with a tracking number before 4:30pm on a business day, an address will be sent to you that day. If we receive the tracking number after 4:30pm or on a weekend, an address will be sent the next business day. You can send the tracking number to dru@texashomebase.com.
- If you do not send us a tracking number, the address will be sent as soon as we have received the physical form.
This form requires of two forms of ID, one of which must be a photo ID. These IDs can be sent via email to dru@texashomebase.com or you can include a paper copy of the IDs with the original 1583 form. Acceptable and unacceptable forms of ID are listed on line 8 of the 1583 form.
Spouses may complete one USPS 1583 form with both names listed on lines 2 & 6. We will need 2 forms of ID for each spouse (one photo ID and one secondary ID).
Any other adult(s) receiving mail at your new address must complete a separate USPS 1583 form and submit 2 forms of ID (one photo ID and one secondary ID).
Social Security cards, credit cards, and birth certificates will not be accepted.

An address can not be sent to you until Texas Home Base has received either the physical form or a tracking number proving that the form has been sent to Texas Home Base.
